Social Worker Long Term Care

Bear Mountain Healthcare is looking for an experienced, motivated, and creative Social Worker for our West Springfield, MA location. The Social Worker answers to the Director of Social Services and the Administrator. The Social Worker's responsibility is to provide for the physical, psychological, and social needs of the residents and all other duties assigned. Additionally, the SW shares a responsibility toward creating and sustaining an environment that humanizes and individualizes each resident.

Job Duties:

  1. Review facility policies and procedures as part of the facility’s interdisciplinary team to assure compliance with state and federal regulations.

  2. Participate in reviewing and setting policies concerning resident care and quality of life.

  3. Participate in developing facility social work policies.

  4. Develop, maintain, and utilize a listing of current community resources that are useful to residents and their families/significant others.

  5. Participate in quality assurance interdisciplinary team meetings.

Hours: 40 hours weekly - 7:30a - 4pm

Days: Monday - Friday

· Ongoing education with free CEUs

Requirements:

  • Licensed social worker in the state of MA

  • Experience in Long Term Care/Nursing Home preferred.
