Job Summary and Qualifications

As a Registered Nurse for Admission and Discharge, the top priority is delivering exceptional patient-centered care. They are dedicated to ensuring a seamless and efficient transition for patients coming in and out of acute care hospital settings. Specialized assessment skills help ensure that patients are placed in the appropriate care areas and discharged. We offer education and post-hospitalization care as necessary. Their main focus is advocating for patients, families, and caregivers while embodying the organization's vision, mission, and values. The goal is to give an unmatched patient experience and attain optimal clinical outcomes.

What will you do in this role:

  • Obtains a nursing assessment history, and ensures proper inpatient care unit admission. Takes appropriate action when needed, such as communicating special circumstances to physicians and/or care team or reporting indications of abuse or neglect.

  • Completes and facilitates the completion of required documentation, such as admission assessments, risk assessments, medication reconciliation, discharge paperwork, and post-hospital documentation.

  • Teaches patients/families/caregivers about a patient’s medical condition, patient status, treatment plan, medications and possible side effects, and follow-up measures. Translates complex medical terminology to ensure complete knowledge.

  • Completes extra discharge documentation, education, and discharge care for special patient populations, to ensure compliance with core measures for discharge.

  • Ensures patients /families/caregivers receive copies of discharge paperwork and any other documentation needed, such as referrals for ongoing care, educational materials, etc.

  • Coordinates with House Supervisors/Nursing Administration to prioritize workload and identify greatest need areas to ensure smooth patient transitions.

  • Demonstrates a time-conscious awareness and ability to prioritize. Distinguishes between low and high-priority activities.

The Medical Observation Unit registered Nurse is responsible for the coordination and care delivery to the assigned patients in order to provide high quality patient care and services. Patient ages range from adult to geriatric. The registered nurse functions as an extension of the charge nurse and the unit supervisor to ensure a professional level of care is maintained. The registered nurse is responsible for the delegation of those patient care tasks that fall within the scope of care for LVNs and PCAs on the unit. The registered nurse has a primary function in the development of a plan of care for the patients and the provision of educational needs for patients, families, and significant others. The registered nurse maintains a high level of competency in the provision of nursing care for patients served.

Minimum Work Experience:

At least one year of recent clinical nursing experience is preferred.

Acute care experience in a medical-surgical, or telemetry acute care setting.

Licensure/Registration/Certification:

-Current ACLS (preferred)

-Current BCLS (required)

-EKG/dysrhythmia recognition (recommended)
