Job Summary

Your main role is ensuring every patient gets uninterrupted care from our healthcare team. You'll coordinate patient care seamlessly across different stages, working with providers to achieve our priorities.

What You'll Do

  • You will review reports to find patients needing adult preventative, diabetes, and other clinical services. You'll use patient records to create reports for care coordination.

  • Coordinating care is a team effort. Work with various teams and resources. Help find community resources for those in need.

  • Customer service is essential. You interact with patients, recommend services, schedule appointments, and assist with paperwork. Resolving concerns for patients and families is also part of your role.

  • To help patients manage health, provide approved education and telephonic engagement to ensure adherence to care plans.

  • Ensuring smooth patient transitions is crucial. You'll monitor patients transitioning between hospital, home, and their primary care providers. This involves completing follow-up tasks and updating care providers on critical issues.

Qualifications

  • Possess a high school diploma, GED, or equivalent education

  • Have under one year of relevant professional experience
